BAPETEN Ensures Licensing Compliance for Radiotherapy and Nuclear Medicine at Siloam and Hermina Hospitals

The Nuclear Energy Regulatory Agency (BAPETEN), through the Directorate of Licensing for Radiation Facilities and Radioactive Materials (DPFRZR), conducted verification activities for the operating licenses of radiotherapy facilities at Siloam Hospital Surabaya, Surabaya City, East Java, and nuclear medicine facilities at Hermina Hospital Bekasi, Bekasi City, West Java Province, from December 8-12, 2025.

The verification of radiotherapy and nuclear medicine facilities was carried out to ensure that radiation shielding, including bunkers, met safety requirements for workers, patients, the public, and the surrounding environment. During the process, the team assessed the conformity of the constructed facilities with safety standards, covering radiation shielding calculations, the readiness of primary and supporting equipment, human resource competence, and the implementation of operational procedures. The assessment encompassed radiotherapy services utilizing Linear Accelerators (LINAC) and nuclear medicine services employing PET-CT, SPECT-CT, and therapeutic nuclear medicine, ensuring all activities met radiation safety and source security requirements.

The verification of the radiotherapy facility at Siloam Hospital Surabaya was conducted by a BAPETEN inspection team led by Herry Irawan, a radiation supervisor from the DPFRZR Health Facility Licensing Group, along with his team members. Meanwhile, the verification of the nuclear medicine facility at Hermina Hospital Bekasi was carried out by a BAPETEN inspection team led by Ahmad Maulana, a radiation supervisor from the DPFRZR Health Facility Licensing Group, along with his team members.

The operating license verification for the radiotherapy facility at Siloam Hospital Surabaya focused on the use of a Varian TrueBeam LINAC unit. Meanwhile, the verification of the in vivo diagnostic nuclear medicine facility at Hermina Bekasi Hospital covered the use of a Siemens Symbia Pro.specta Q3 SPECT-CT scanner utilizing I-131 and Tc-99m radioisotope sources, as well as a United Imaging uMI550 PET-CT scanner using an F-18 FDG source. Additionally, verification was conducted on the therapeutic nuclear medicine facility, which utilizes I-131, Sm-153, Lu-177, and P-32 radioactive sources.

The scope of the on-site inspection included a review of documentation and the technical conformity of the constructed facility's design, including the adequacy of radiation shielding thickness in the radiotherapy bunker. For the nuclear medicine facility, verification encompassed areas for radionuclide or radiopharmaceutical preparation, counting, and storage (equipped with fume hoods); radiopharmaceutical administration; patient imaging and observation; decontamination; radioactive waste temporary storage and pre-disposal processing; patient-specific waiting areas and restrooms; and isolation inpatient rooms for therapeutic nuclear medicine. The team also assessed the performance of primary and supporting equipment, conducted radiation exposure measurements and dosimetry testing, and evaluated patient workflows, personnel competency, and radioactive source protection and security systems.

Verification results demonstrated the hospital's commitment to addressing the BAPETEN team's findings within the stipulated timeframe. The verification process proceeded smoothly with the hospital's full support, and all feedback served to enhance radiation safety compliance in accordance with applicable regulations. With the issuance of the operating license for the LINAC unit at the Mochtar Riady Center for Advanced Care (MRCAC), it is expected that Siloam Hospitals Surabaya’s radiotherapy services will further expand access for the people of East Java and Eastern Indonesia to global-standard healthcare; meanwhile, the nuclear medicine and molecular theranostics services at Hermina Bekasi Hospital will facilitate public access to advanced medical technology, particularly in the field of oncology.
