Verification of Nuclear Medicine Operating Licenses at RSUP Surabaya and Awal Bros Hospital Pekanbaru

The Nuclear Energy Regulatory Agency (BAPETEN), through the Directorate of Licensing of Radiation Facilities and Radioactive Materials (DPFRZR), conducted simultaneous verification activities for nuclear medicine facility operating licenses at the Central General Hospital (RSUP) Surabaya, in Surabaya, East Java, and Awal Bros Hospital Pekanbaru, in Pekanbaru, Riau Islands, from August 11 to 15, 2025.

The verification activity aimed to ensure that the nuclear medicine facilities were safe for workers, patients, the public, and the environment. The verification team confirmed that the constructed facilities met safety standards by evaluating radiation shielding designs and key equipment, supporting equipment, human resource competence, and operational procedures. This assessment covered operations involving Single Photon Emission Computed Tomography (SPECT-CT) using I-131 and Tc-99m sources; Positron Emission Tomography (PET) and Computed Tomography (CT) using F-18 FDG radiopharmaceuticals; and nuclear medicine therapy using I-131 sources, ensuring compliance with radiation safety and source security requirements.

The BAPETEN inspection team for the RSUP Surabaya facility was led by Roy Candra Primarsa (Activity Manager, Health Facility Licensing Function Group, DPFRZR BAPETEN) and included other team members. Meanwhile, the inspection team for Awal Bros Hospital Pekanbaru was led by Made Pramayuni (Senior Radiation Inspector, Health Facility Licensing Function Group, DPFRZR BAPETEN) and included other team members. Verification activities for the operating license of the in vivo diagnostic nuclear medicine facility at RSUP Surabaya covered a GE SPECT-CT scanner (model NM/CT 870 DR) using I-131 and Tc-99m sources, a GE PET-CT scanner (model Discovery MI Gen2) using F-18-FDG, and a therapeutic nuclear medicine facility using I-131 sources. Meanwhile, at Awal Bros Hospital, Pekanbaru, the verification covered a Siemens SPECT-CT scanner (model Symbia Pro.specta Q3) and a therapeutic nuclear medicine facility using I-131 sources.

The scope of the on-site inspection included document review and technical verification of the constructed facility's design compliance and the adequacy of radiation shielding thickness in the radionuclide/radiopharmaceutical preparation, counting, and storage areas (equipped with a standard-compliant fume hood), as well as the radiopharmaceutical administration room, patient imaging room, post-administration patient room, decontamination room, radioactive waste temporary storage and pre-disposal processing area, and dedicated patient restrooms for use after radiopharmaceutical administration. Furthermore, the process involved verifying the performance of primary and supporting equipment, measuring radiation exposure, ensuring proper patient flow and personnel competence in operating the SPECT-CT and PET-CT scanners in accordance with standards, and inspecting radiation protection equipment and radioactive source security measures.

Following the nuclear medicine facility operating license verification, the hospitals committed to addressing the findings presented by the BAPETEN team within a specified timeframe. The verification process proceeded smoothly, supported by the hospital staff who actively participated and welcomed the proceedings; any identified deficiencies were regarded as technical input and recommendations for achieving radiation safety compliance in accordance with applicable regulations and standards.

The issuance of the nuclear medicine facility operating license for RSUP Surabaya will enable the provision of comprehensive, state-of-the-art nuclear medicine and molecular theranostics services capable of detecting cancer with high precision.
