Focus Group Discussion on Updating the Indonesian Diagnostic Reference Level for General Radiography, CT Scans, and Fluoroscopy

The Nuclear Energy Regulatory Agency (BAPETEN) held a Focus Group Discussion (FGD) on Friday, August 1, 2025, to update the Indonesian Diagnostic Reference Level (TPD). This was the second FGD to update the Indonesian TPD, specifically for general radiography, CT scans, and fluoroscopy.

The online FGD was attended by representatives from the Ministry of Health; Nuclear Safety, Metrology, and Quality Technology Research Center - National Research and Innovation Agency (PRTKMMN - BRIN); Indonesian Hospital Association (PERSI); various professional organizations and educational institutions, such as the Indonesian Association of Dermatologists and Venereologists (PDSRI), Indonesian Cardiovascular Specialist Association (PERKI), PIKI, Indonesian Medical Physicists Alliance (AFISMI), Indonesian Radiographers Association (PARI), Universitas Indonesia, Poltekkes Semarang, and technical work units from BAPETEN.

The Head of Centre for Regulatory Assessment of Radiation Facilities and Radioactive Materials (P2STPFRZR), Dedik Eko Sumargo, in his opening remarks, emphasized the importance of updating the Indonesian diagnostic reference level values. "The agreed-upon Indonesian TPD values will be published at the end of August and launched on September 3, 2025, in conjunction with the Radiation Safety Seminar on Medical Exposure (Sekarpadi)," he said.

The event continued with an introduction from Rusmanto, the Activity Manager of the Health Assessment Functional Group. He highlighted the need for agreement on the proposed national TPD values, rescheduling the next review, improving data by comparing the United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) and AFISMI, following up on the agreement on examination protocol names, and adding data, especially for specific examinations or pediatric examinations.

The main topic was presented by Endang Kunarsih, Lead of the Indonesian TPD Implementation Working Group. She presented a profile of the results of the Indonesian TPD analysis for general radiography, CT scans, and fluoroscopy.

In addition to presenting the latest developments, this FGD also discussed the results of the first FGD held previously. The first FGD agreed on the importance of updating the Indonesian Diagnostic Reference Level (TPDI) values established for 2021–2022. One concrete follow-up measure is a proactive approach to healthcare facilities to accelerate and expand patient dose data collection.

The data collection has increased by approximately 40 percent. This data is now being used to calculate more representative TPD values.

The Indonesia 2025 TPD Team also presented the latest proposed TPD values for various types of CT scans, general radiography, and fluoroscopy. Compared to 2021 data, most CTDI (Computed Tomography Dose Index), DOS (Dose Length Product), ESAK (Entrance Surface Air Kerma), and INAK (Incident Air Kerma) values have decreased. This indicates a direction for improvement in optimizing radiation doses for patients.

For example, in contrast-enhanced CT abdominal examinations, the 2021 CTDI value of 20 mGy has been reduced to 14.1 mGy, and the DLP value has been reduced from 1360 to 1069. Meanwhile, in general radiography, such as chest radiography, the ESAK value has been reduced from 0.4 mGy to 0.24 mGy.

In addition to comparing with Indonesian data, the team also presented international benchmarking from countries such as Australia, Korea, Japan, and Germany. This is part of an effort to keep Indonesia's TPDI in line with international standards. Furthermore, the establishment of a national TPD value will lead to positive acceptance and appreciation at the international level.

The FGD also emphasized that the Indonesian TPD is not a maximum patient dose limit, but rather an evaluation tool to optimize radiation exposure so that patients receive doses according to their needs.

The FGD agreed that the proposed values would be accepted as the next Indonesian TPD values. In addition, several matters that must be followed up include the need for special discussions to reach agreement on procedures or nomenclature for examination types; consideration of different types and brands of modalities; imaging technology; setting standard dose indicators in modalities; ensuring traceability of radiation dose measurements; and developing coaching patterns through workshops, education and training, and technical guidance on dose recording, dose calculation, dose reporting, dose analysis/audit, and patient dose optimization.
